Medical Social Worker I
Position Summary:
Provides social work services to patients, families and identified caregivers as part of an interdisciplinary care team.
Responsibilities:
- This includes conducting psychosocial assessment, intervention and counseling with a particular emphasis on transitions of care in a healthcare setting
- Maintains current knowledge of resources available and shares this information with patients, families and identified caregivers
- Collaborates across the organization and with community stakeholders
- Works under broad supervision
- Performs other related duties as assigned
- Familiar with standard concepts, practices, and procedures within the field
- Regular and predictable attendance is an essential job function
- Competent to meet age specific needs of the unit assigned
Requirements and Minimum Qualifications:
- Master’s degree in Social Work required
- Licensure and/or certification as required by the State of Idaho; Idaho license by endorsement must be initiated by time of hire and obtained within 3 testing cycles, not to exceed 1 year of employment
- 1 year of social service experience in a health care setting or social services agency preferred
- Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). Compliance with accepted professional standards and practice
Working Conditions:
- Must be able to maintain a standing and/or sitting position
- Typical equipment used in an office job
- Repetitive movements
