Supports the psychosocial needs of Kootenai Health patients within the outpatient setting, working towards enhancing patient safety and wellbeing. 


  • Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle

  • Administers standardized psychosocial assessments and coordinate supports applicable to patient and family needs

  • Conducts financial screenings and connects patients to internal and external funding resources

  • Assists patients and families with advance healthcare planning, including Living Will and Durable Power of Attorney for Health Care and POST forms 

  • Assesses for complicated grief and anticipatory bereavement related to medical condition and aspects of loss and adjustments along the continuum of illness and care 

  • Assists patients and families in coping with serious medical conditions

  • Collaborate with doctors, nurses and multidisciplinary team members to meet patient needs 

  • Works under general supervision of Social Services manager

  • Performs other related duties as assigned

  • Familiar with standard concepts, practices, and procedures within the field

  • Regular and predictable attendance is an essential job function

  • Competent to meet age specific needs of the unit assigned

Team Highlights

Our Outpatient Social Services team is dedicated to providing care and assistance to patients with psychosocial and mental health related needs in outpatient clinic settings for Kootenai Health.  

 
 

Requirements and Minimum Qualifications:


  • Bachelor’s degree in Social Work required; Master’s degree preferred

  • LBSW as required by the State of Idaho completed within 6 months of hire

  • 1 year social service experience in a health care setting or social services agency preferred

  • Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). Compliance with accepted professional standards and practices

  • Must be able to maintain a standing and/or sitting position

  • Typical equipment used in an office job

  • Repetitive movements
